Another form of the Cause/Effect Analysis, this deductive tool begins with a conclusion, then attempts to determine the specific causes of this conclusion. It provides a graphical representation of the combination of failures and/or errors and the most common causes that result in a condition or occurrence. It also helps to identify the risk drivers — those choices, causes or conditions that have the most impact on the risk, as well as the risk drivers that affect numerous risk factors.
